I need some advice on a code. My provider performed a change of Chait appendicostomy with fluoroscopy and the code that is being used is 49450. I think there should be an unlisted code 44979. See op note below.

Procedure:
The patient was identified and placed in the supine position on the operating table.  Adequate anesthesia was induced.  Timeout was performed and all present were in agreement.  The existing Chait tube was removed with steady traction and after placing the obturator and the new tube this was directed into the appendicostomy remove the obturator leaving the Chait tube in place.  The tube was instilled with 50 mL of Omnipaque 240 with fluoroscopy which demonstrated no extravasation and good filling of the cecum.  Another 50 mL of saline was placed into the cecostomy tube following the contrast into the transverse colon.  Minimal stool was noted.  The colon was less distended than previous.  The procedure was well tolerated, and there were no complications.
