Can anyone give me any direction for 93657 that now has MUE of 2? Can it be billed for CFAE ablations after PVI even if patient is not in Afib?
Follow 93657 x2 after AF ablation 96356 with both a "posterior wall ablation procedure" and an "anterior mitral Line"?
 
The AMA addressed CFAE ablations in CPT Assist September 2019 (they have stated it is appropriate to report 93657 for CFAE ablation even if the patient is not in afib after a PVI). That was a change to prior guidance/understanding of the intent of code 93657, but since that guidance came from the AMA, most coders/consultants are now following that guidance. For your second scenario, it depends on whether they are treating persistent afib in those areas (if they complete PVI and then there is persistent afib requiring a posterior wall ablation for example, you could add a unit of 93657 for the posterior wall ablation). If they are just extending ablation lines to complete the PVI (might be more applicable with that anterior mitral line), then it would be part of the 93656. So definitely depends on the documentation, but hope that helps a bit :)
