Good Afternoon,

I would be ever so appreciative if someone could direct me. Our physician is wanting to charge a fee for the detailed paperwork. The physician is currently treating the patient and they are requiring paper work to be filled out annually.

Any thoughts on the CPT we should use? I looked at 99455 but I was unsure when these codes are appropriate. Would be 99080 be more appropriate?

We have an occupational department and 99455 is the best option to bill out. You will be billing the companies and not the insurance, correct? We charge $50 for a DOT/Work Physical.
