I want to audit charts for proper documentation/charting, how many charts would be a good sample?
 
Audit Cycle

Ten encounters per provider per month is a usual place to start. Providers who are 90% and above should be considered passing and warrant annual audits. Those who are less than 90% should be audited on a cycle consistent with their accuracy (lower scores are audited more often). A good rule is to audit those under 50% monthly, 60-70% bimonthly, and 80% quarterly. Keep a spreadsheet of who was audited when, with what score, and the focus of the audit both for your own records and the ultimate interest of your management/physicians. Eventually, they'll want to see trend lines.
