I am wonding if anyone out there would be willing to share chart notes for a 99213 and a 99214. I don't feel like our office is capturing all that we should and we are not documenting properly. Of coarse I would expect the pt info to be removed.... looking to see if there is something more we could be doing. Anyone willing to help or direct me to a sample site? I've read all of the point system info which is a bit puzzling.....

I highly recommend emuniversity.com - wonderful website with many examples of efficient documentation that still captures all that is needed for level 3 or 4.
