In Regards to CPT Code 99238 and 99239
Can patient discharge planning, rxs, care guidelines, etc and final charting in EHR occur outside of the Hospital Facility. Upon reviewing charts for our hospitalists when looking for time documentation the charts are all completed outside of provider being present in the actual facility.
I have read that the time quantification for these codes should be based on total unit floor time for services rendered while in that facility. For example the provider shouldn't count dictation for inpatient service toward time if it is performed at your outpatient office setting.
Does this change since EMR EHR has come about, Are there better specifications regarding charting to help offset time stamps of EMR EHR technology. Any guidance would be greatly appreciated.
