Is there an updated code that Medicare/Medicaid uses for chelation therapy? I saw an S code for home health that is specific to BCBS, but am researching for Medicaid. I think M0300 was previously used, but is deleted. I don't know if CMS has deemed this service as non payable. I know it would be covered for certain conditions and certain drugs if there is true lead poisoning or other severe conditions. It was used fraudulently in the past by providers who would determine "heavy metal toxicity" by urine tests, which are inaccurate. It used to be denied by insurance BCBSFL, but now some plans cover it.
 
You're right - the M code is not covered by Medicare. However it appears from just searching for medical policies from other insurances that you would code it like a normal infusion.
