Hi,

I'm new to coding out patient and need some help.
When using code 17250 is the code used once for each antomical site or per ulcer?
For example, would I code cauterization of two thigh ulcers and three cauterization of foot ulcer 17250 x2 (thigh) 17250 x 3 (foot) or would I just be use the code 17250 once for each anatomical site?

Any references for the answer would greatly be appreciated!
