The 96413 represents the infusion that runs 16 min-60min. If you want to bill a 96415, the drug needs to run at least 31 min.

I think what she meant was the the infusion needs to run 31 minutes over the 60 minutes. It has to run atleast 91 minutes to count an additional hour.
