Top 4 are chemo/complex drugs. Pepcid on down are supportive (therapeutic). First, I'd list this out by time. It makes it easier to read and code. Second, are you in office or a facility? Use the guidelines and code descriptors to guide you. Which part are you getting stuck on?
 
Hi Not sure if you got a detailed answer to your infusion. I have coding Oncology office and outpatient clinic including infusion center for about 12 years.
Based on the documentation in the treatment cycle above the coding would be as follows:
96413 x1 first hour of Truxima (312 mins total)
96415 x4 for the additional 4 hours of Truxima
96417 x1 for the Cytoxan (43 mins)
96411 x2 for the Adriamycin and Oncovin (both are pushes)
96367 x1 for the Emend (26 mins)
96375 x3 for the Aloxi, Benadryl and Decadron (all pushes)
96368 x1 for the Pepcid (ran 30 mins concurrent to the Truxima) note your organization may or may not wish to bill the 96368
