I have a PM&R physician who is inquiring if Phenol is considered chemodenervation. I have not been able to find any credible sources to clarify his inquiry. Does anyone have information regarding chemodenervation vs. destruction by neurolytic agent? Also, code 64640 can no longer be billed with add-on code 95873 does this mean the electrical stimulation for guidance is included in 64640 since the change in 2014?
