For CPT code 96409, could this be used for Vincristine IVP administrations when the time is documented as 20 minutes and nothing supports that the nurse was at bedside while pushing the medication? It is my understanding that this medication should not be given without direct nursing supervision and by the dosage, it will take around 20 minutes to appropriately administer. I am not sure if we can use clinical judgement or should there be a policy on this type of administration?

If the order for the vincristine was supposed to be administered as a push, then it must be charged as a push regardless of how long it took. I would ask for the doctor's order or check with the nurse to see if it was in fact pushed or was the bag hung and ran for 20 minutes.
