When the assistant notes the reason why the patient is here today, the assistant will first document the provider seeing the patient. Should this be noted in the chief complaint? Would this be unacceptable in an audit?

Hello,
I don't see an issue, as long as the information is accurate and it is the physician/provider seeing the patient.
