I need some help in finding ICD 9 code. A mother brought her young daughter into our office for a note to excuse her from swimming due to chlorine allergy. She has not been swimming so there are no symptoms. Nothing is medically wrong with her. She just needed a note for school. Any suggestions?
 
There is a V code for administrative purposes (i.e., school forms, letters, etc). It's in the V70 section...I think it's V70.3.
