Our group of coders has a disagreement regarding picking the correct Hyperlipidemia code. Our Physician states in plan "she has good lipid control other than a mild elevation of TG's" She is on Meds for her hyperlipidemia. the question is can you pick say E78.2 based on this information or does the Physician have to actual state she has mixed hyperlipidemia? Thank you for your help.
