I'm wondering what code I should use for the chronic anticoagulation. There is no mention of hemorrhage and the patient is on Coumadin. 790.92 shouldn't be used for patient's on Coumadin Therapy.

Any thoughts would be greatly appreciated!

790.92 is for an abnormal coagulation profile not for Coumadin therapy.
You would use v58.61 long-term (current) use of anticoagulants to code current Coumadin use. If the physician documents that the coagulation profile is abnormal then you could code it. In the ICD 9 under the code in the tabular it gives examples of what constitutes abnormal or prolonged.
