Hello!

I am working with a NP in NJ that treats patients in assisted living, group home and home bound patients. He wants to start billing for Chronic Care Management but we are not exactly sure how that has to be documented in a note. Does anyone have any information on CCM documentation when billed by a provider and not the office staff?
