Hi,

On this code 99490 would anyone be able to tell me what details should be provided in the documentation? Does the documentation have specific to the Chronic Condition diagnosis (es) or can it be general (doesn't speak to any of the Chronic Conditions? Is a Medical Assistant qualified to conduct this call?
