When can we capture Chronic conditions from Past Medical History? If chronic they should be evaluated in each encounter with the patient, but if that is not the case, can we still do it?
Ex: PVD in PMH, Physical examen is not relevant and there is not treatment.

The rule on chronic conditions is, we code it if it is managed, controlled, or treated, or if it is a co-morbid condition for the problem being treated.
