what is your take on coding chronic migraines? my docs don't ever state chronic, but will elaborate in the history that the patient has a longstanding hx of migraines. based on the history, am i allowed to code a chronic migraine, or only use unspecified migraine? i know i can always query the doc, i just wanted another opinion.
