I have a question regarding correct coding for a patient who has been diagnosed with Chronic Rhinitis. The patient presented to the office with itchy, water eyes, post nasal drip, and sinus pressure. The physician selected Chronic Rhitinis and Post Nasal Drip as the diagnosis, however, from what I understood if a symptom is part of a specific condition, you code the condition only; not the condition and the symptom unless that symptom is unrelated to the condition that was diagnosed.
Is this correct or am I missing something?
