Hello,

I'm looking for input concerning diagnosis coding for "chronic skin tears" for a nursing home visit. Provider documents that skin tears are ongoing. I'm coming up empty with "chronic," "history of," and "frequent."

Also, what would I enter for date of onset/injury when I bill this?

Thanks in advance for any replies!
 
Code open wounds by location. The chronic nature is that these wounds are either not healing or are frequent because of the patient's compromised skin turgor, which is not really codeable.

Date of onset would depend on the wound being treated that day.
