Wiki Citation needed coding from body of op report not procedures performed

Good Day everyone,

The provider notates :
PROCEDURE PERFORMED
1. Excision of left upper back mass, final excision size 10 x 14
cm.
2. Complex wound closure in layers of 10 cm incision.

In the body of the op report, he documents the closure, but doesnt indicate the size. I'm being told to take the size from the procedures performed section. Can anyone provide official citation that says I can/can't. Everyone says you don't code from procedures performed. I need support from an official source to take back to the provider.
