Can more than 1 physician perform a 99304 (assestment in a nursing home) on the same patient? I am working with a Physical Medicine physician who is asked to evaluate patients in a rehab nursing home. These patients sometimes have multiorgan system failure and he is asked to evaluate them for PT/OT (which he does not perform) and coordination of care. The PCP is asking for his evaluation and it could be well after the patient is admitted. So the PCP has already performed his evalutaion (do we need to add the AI modifier for Medicare to the PCP, that's another question) and my physician is performing an additional evaluation from his perspective.
