Question 1: - Patient is diagnosed with Chronic kidney Disease Stage 5 and is on Dialysis - can we assign N18.6 and Z99.2?
Question 2:- Patient has Chronic Kidney Disease Stage 4 and is on dialysis - can we assign N18.4 and Z99.2?

Yes coding is correct above for Question2.
Question 1 If pt has CHKD stage 5 use dx N18.5 and Z99.2 . Also if given End Stage Renal Ds CHKD then assign N18.6 dx. and Z99.2 & Z94 if one kidney is missing.
