Wiki CKD due to HTN and the DM "with"

Ok question.
Now that 81CKD and DM are linked by a with, and HTN always has been, is it appropriate to code DM due to HTN AND e11.22? I would assume it would be like a cataract and as long as it is not specified as not linked it would. HTN is assumed, so why wouldn't DM and CKD be as well, even though it is specified as HTN? I mean the relation is linked. Does it matter if the provider says it?

Using the guidelines for the tabular “with” and code also notes, I would code as I12.9, N18.9, E11.22.
 
DM in index does not have a direct link to HTN but does have a direct link to CKD. Could you please explain how you linked in the index under "with" new guideline. Thank you
 
Under CKD it states to code first any associated diabetic chronic kidney disease and hypertensive chronic kidney disease. Ex: i12.0, e11.22, n18.6
 
thank you Devin, My confusion was linking the HTN with DM. I was indexing DM with HTN not HTN with renal (CKD). So, it's a 2 step approach, not a one step DM with HTN. Thank you for clarifying.
