Hey all - can anyone help with clarification on these codes, specifically whether or not the provider has to exam the patient on the date of discharge? The guidelines state "as appropriate" - but what does that actually mean, that it's NOT required? Thanks in advance.
 
While a physical exam is not required to be documented for discharges summaries, it does help to support a face-to-face interaction between the provider and the patient which is required. When I educate my providers, I suggest that if they are not going to document an exam, that they clearly document a face-to-face conversation with the patient.
