Our biller asked me about documenting Class A/B/C Findings (Q7,Q8,Q9) for patients with Medicare and diabetes. Can anyone tell me where I find this information and how to use it? Thanks!
 
The information is listed within the medical record--generally the examination area. The appropriate modifier is appended according to the criteria for each. The HCPCS manual has some information on these modifiers and Medicare has a manual on diabetic foot/wound care that explains the requirements of each modifier and associated medical record documentation.
