A patient came in to be cleared for Biolife. At the exam at Biolife they noticed her proteins were abnormal so they sent her to her PCP to fill out paperwork. The provider also assessed her depression and anxiety.

How would this be coded? 2 separate claims one with e/m and the other with the paperwork? We do charge for forms to be filled out.

Any thoughts would be appreciated!
