Wiki Clinic Psychologist Evaluating for SCS

Can someone lead me into the right direction? I am having a hard time figuring out what sequence to code for a patient that is being evaluated by our clinic Psychologist to see if they are a candidate for a spinal cord stimulator for chronic pain. Any help would be appreciated.
