A hospital based clinic seeing patients with (many) chronic conditions assigns one of their pharmacist employees to that clinic to help with medication issues advising physicians on drug selection, providing medication histories and medication reconciliation services for patients who see outside physicians as well as those in the hospital clinic. Can the hospital bill for these services, and if so what codes are appropriate.
