Clover is retracting tons of claims for the modifier used. I bill for a ambulatory surgical center. We bill 45380 with 43239 51 modifier. Clover is denying for the procedure code is inconsistent with modifier used 835 Healthcare Policy Identification Segment denial code actually means, and what loop 2110 REF. Has anyone else experiencing these retractions? Every other carrier we bill including Medicare process with no problem.

When coding this out, I'm not being prompted for a modifier. If you know for sure that you need one, maybe try an XU modifier instead of the 51?
