I have an acquaintance who has a small physical therapy practice, only 1 provider. We have a question about the form line #32. The question is must this field be completed along with field #33 in order for form to be completed accurately. She performs many services in the home, and that field does not allow her to add her NPI. Hence we want to be sure that it is fine to just complete the Billing provider Information and Phone
