I am new to Medicare claims and have a Humana Medicare remit with several patients that are denying the 2nd and 3rd CPT billed as CO102 Major Medical Adjustment. I can't find any definitive answer as to what this denial means. Before I process this remit, I want to make sure I understand this denial fully. These claims are for an ASC.
