Hi! I'm a Medicare and Medicare replacement biller. I'm working my denials and am coming across a lot of claims with CO 18 denials. The thing is, there's no indication that this claim has been billed before. The very first EOB received says the claim is a duplicate. Is there a reason why these claims would deny as duplicates when they're really not duplicates? Thanks in advance!
