It is usually considered 88304, akin to sinus polyps, or a soft tissue debridement if the contents were removed to clear obstructions to breathing (i.e in cases of OSA). It all depends on the circumstances, though. If it is a biopsy that does not reveal polyps, and the clinician is asking for evidence of eosinophilia, it may be more correctly coded as 88305. If you read your CPT options literally, it should become clear which code is correct.

88311 is used for decalicification of bone. Sometimes the sinus contents will contain turbinates that may need to be decalcified. If Decalcification is documented in the report, then 88311 may be appropriate.
