Wiki Code 99053 with ED E/M code

We are not sure if we should be using modifier 25 with code 99053. Most of the payer's are not paying it saying that it is bundled in with the e/m code but if we do a review and add the 25 some of them will pay it. I'm not sure if it is Correct Coding Procedure to add the modifier or not.
