I need advice on how to code the following scenerio as documented in notes. Patient presents to monthly B12 injection by the nurse and she is taught to do injection herself. Patient appeared to understand the technique and successfully injected dose into left thigh. Originally billed as 96372, J3420 and 99211/25 all with diagnosis of 281.0 by office. Insurance is denying 99211/25 as inclusive to 96372. Is there another diagnosis or CPT code that we code use for the instruction for self injection of B12? Is this instruction even a separately billable service?
