Wiki code hyper or hypothyroid??

I have a provider that keeps coding hyperthyroidism on patients who are on hypothyroid medication but because their dose is off, they've become abnormally suppressed. Should I actually be coding hyperthyroidism, or just hypo since that is what we were originally treating and we're still keeping them on hypothyroid medication?
 
If they were taking the medication according to direction then it is an abnormal lab as an adverse reaction. If they were taking the medication incorrectly then it is a poisoning. An isolated elevated lab is not the same as having the condition.
