I AM GETTING A DENIAL FROM MEDICARE (CO -4) - a modifier is needed. And i am wondering what modifer to use, because on ingenix coder there are so many to choose from. Please help!!!!!!!!!!!!!!!!

check the LCD

Marty,

Have you checked the LCD for that code? I don't know which Medicare region you fall into, but there should be an LCD you can reference for that code for your region. Hope that helps.
