If I am reading the AHA Coding Clinic 1Q,2012 correctly, does this mean that if the physician chooses a diagnosis from the EMR and the code is listed with the descriptive term in the assessment, that it should not be used? Is this the same as listing a code number and not complying with the doctor using a written diagnostic statement in the assessment.
At my facility the doctors choose their own codes and they are listed with ICD-10 codes followed by the description of the code. Just like in our ICD-10 book.
