Patient undergoing chemotherapy for breast malignancy experienced extravasation when the IV became dislodged, leaking the chemotherapy agent into her tissues. What is the primary diagnosis?

Extravasation

When I look up extravasation, as I am sure you did, I am taken to code 999.81 Complication of medical care. It says to use additional code where applicable, to identify specific complication. Your note does not say what that is. Was there an adverse effect?
Section 1B18 in the Guidelines has information to read.
You first state that this is a complication, and then what the complication is-what happened to the patient as a result. The chart note is note specific. here. Multiple codes need to be assigned.
