My physician is billing an E/M level based on time: "This visit was 30 minutes with greater than 50% direct care and counseling regarding her upper respiratory symptoms, hypothyroidism, hepatic cyst,kidney angiomyolipoma and preventative medicine."
If he is to bill for smoking/tobacco use cessation with 99406 (3min-10min) how would you suggest he document the time for the cessation along with the time for the E/M?
 
I hope this helps.

Any little more detailed documentation, such as what type of discussion took place etc., would be more helpful to bill the 99406 code. I would also document the time spent discussion the smoking cessation.
