Wiki Codes 99495 and 99496

Good morning! I'm wondering if anyone has billed the Transitional Care Management codes 99495 and 99496 yet. There was an article in the April 2014 describing the codes, so I mentioned it to the doctors that I work for and they want to use these codes, but I don't have 100% of the details. For example, I work for Nephrologists, so can they use these codes or can only the primary doctor use these codes? If the primary care uses them, can the consultant use them as well? Also, are these codes billed one time at the end of the month that they are used in? Thanks in advance for any help you can send my way!:)
