Does anyone know if a patient completed questionnaire with a separate interpretation and report from a physician (non Psychiatrist or Psychologist) is billable and if so with what codes? This is a non-medicare case.

The patient completed questionnaires can be a screen/assessment for the patients pain, and/or medications, and/or functional abilities. My initial thought is to incorporate the physicians time reviewing and analyzing the information into his or her E&M in some form or fashion, but I am not sure.

Any thoughts anyone??
