The code includes one or both tubes and/or one or both ovaries. If it is only one side or both you report the same code. But that also means that the payer may deny a second code for removal of cyst (58662) even though there appears to be no bundling issues.
 
I agree. Both codes may be billed per coding guidelines. However, you may receive an initial denial or request for records. I would ensure to match the appropriate diagnoses to each procedure.
