Wiki Coding 96365 and J2003 together

Does anyone have any advice, suggestions or alternative methods of coding 96365 and J2003 the infusion and the Lidocaine together for Medicare? These claims are being denied because Medicare indicates that the J2003 is bundled into the 96365. Welcoming anyone's input how they may have handled billing these themsleves.

Is lidocaine being administered with another drug or substance? I would imagine that it is actually bundled, if the lidocaine is only being given as a necessary part of another medication administration.
