Wiki Coding a repair of a webbed toe repair

We are seeing a 12 yr old female who at 18mo had a web toe repair preform by a orthopod who use a skin graft from the labia fold. Now the patient toes have become hammertoed because the skin graft did not grow with the toe and the patient has hair growing inbetween her toes. This is causing the patient severe pain to the point she will not let those toenails be trimmed. Our physiscian has to remove the orginal skin correct the hammertoe deformity and place another skin graft to this site. What would be the most appropriate codes to correct this problem?
