my physician is asking can we bill for urine collection and psych screening on dates when we do procedures. (pain specialist)

help me to find principle ICD

36 old lady visited Urology clinic with the complaint of burning maturation and pelvic pain,
doctor diagnosed as kidney infection, cystitis, and liver disorder. during pelvic ultra sound doctor find this lady is 10 wks pregnant,
how we will code this and what will be the principle diagnosis.

You must use a code from Chapter 11 as your first listed code. However you really need to look these up and then propose the codes and I or anyone else can let you know if you are correct

Would the E/M require the 25 modifier? The guidelines are very limited.

Medicare

Medicare denied CPT 88333 billed 3 units for invalid number of units, i checked the NCCI for the correct number on units and it shows # 4 as number if units, does any one knows if the claim has to go as per 1 line per CPT? Thank you
