When providers perform ex. wart biopsy during an EM, do we additionally need them to specify that "[25] minutes were spent with more than 50% in counseling and coordination [of skin care protection] excluding time spent on procedure" to code by time? I believe this to be true but would like more solid back up information. The NCCI edits in Chapter 1 section D is the only info I could find. Anything else would be greatly appreciated.
